I think it's just got its leg curled under it. That may be because it appears to have been sculpted as if mid-crying fit, tears, snot and all.

Sometimes baby boys' willies look larger than they will do a few days after birth. Like with other parts of the body (eyes, particularly) that appear swollen after birth.
